Vitrum RX LLC: Fraud, Waste, and Abuse

Fraud, waste and abuse can happen anywhere, and usually results in higher health care costs for everyone. Some examples include:

  • A provider that bills for services or supplies they never gave you, like charging you for a visit you never had, or a back brace you never got.

  • A provider that charges twice for a service or item that you only got once.

  • A person who steals your ID card and uses it to submit fraudulent claims in your name.

  • A company that offers you a drug plan that hasn’t been approved.

What is FWA?

Health care fraud refers to intentional deception or misrepresentation with the purpose of obtaining unauthorized financial gain or benefit, whether by individuals or organizations. This includes but is not limited to submitting false claims, billing for services not rendered, or engaging in fraudulent practices.

Waste refers to the inefficient use of resources, including time, supplies, or equipment, resulting in unnecessary costs or financial loss.

Abuse refers to practices that are inconsistent with accepted healthcare industry standards, resulting in unnecessary costs or improper utilization of resources.

Reporting and Investigation

Mechanisms: We maintain confidential reporting mechanisms, such as hotlines or designated email addresses, to encourage employees, contractors, and stakeholders to report suspected fraud, waste, or abuse. We ensure that individuals reporting in good faith will be protected from retaliation.

Upon receiving a report or discovering potential fraudulent activities, we will promptly investigate the matter. Investigations will be conducted in a fair, thorough, and confidential manner, with appropriate measures taken to protect the rights of all parties involved.

If you suspect health care fraud, waste, or abuse (FWA) from a pharmacy, provider, or anyone else, report it to Vitrum Rx right away. Vitrum investigates all reports of suspected FWA. ‍
